Locum tenens 101 for physicians: A newcomer’s best guide

Locum tenens is a Latin phrase that means to “hold a place.” In the medical world, it describes physicians and other professionals who contract with a medical facility for a specific amount of time — weeks, months, or even years — before moving on to another locum tenens assignment in a new location.

Locum tenens started in the 1970s as a way to support and share resources with underserved regions of the United States. In many ways, that’s still the primary purpose of these positions. But each year, more and more physicians are choosing locums for both the flexibility and control it gives them over their careers.

Advance your career and explore new opportunities

Used strategically, locum tenens assignments can help you achieve goals that might otherwise be out of reach. Do you need more time for volunteer work? Do you want to experience practicing in another state, region, or even country? Do you want to supplement your current income? Whatever the situation, there’s a good chance you can use locum tenens to open the door to new opportunities. Consider these real-life examples:

  • Dr. James Stone, a hospitalist, uses locums to travel and explore different regions of the United States. He accepts six- or 12-month assignments — just long enough to feel like he really lived in a place and adopted a local’s point of view.
  • Dr. Jenny Martino, a pulmonologist, chooses locums to create space and time to work internationally for organizations such as Doctors Without Borders. During a typical year, she might work six months in a foreign country and three months at home — then take the remaining three months off to vacation, rest, and plan her next year of assignments.
  • Dr. Richard Rothfleisch, a pulmonary critical care physician, works locums because he got tired of running his own practice. After studying the possibilities, he discovered that “I could make the same amount of money doing locums full-time by working 14 to 15 days a month and having the rest of the time off as block time.”
  • Dr. Blaine Cashmore, a general surgeon, occasionally works locums on the weekend to supplement his regular income. His wife and children sometimes travel with him and make a mini-vacation out of the experience.
  • Dr. Ana Zamora, a pulmonologist, has used locums to bridge the gaps between full-time employment. In the past, she’s even used locums to test living in a particular region before accepting a permanent position there.
  • Dr. Sanat Bakshi, an internal medicine physician, chose locums after his retirement when he discovered he wasn’t ready for full-time fishing or golfing. With locums, he can work as much or little as he likes!

Being your own boss

A locum tenens physician typically works as an independent contractor. But what does that mean? What are the expectations, and what are the drawbacks, if any?

In many ways, being an independent contractor means you are your own boss. You choose where you work and for how long. Being an independent contractor also means taking charge of your taxes and providing for your own medical and dental insurance. The total amount you pay in taxes should be similar to what you would pay as a permanent employee; but you’ll need to create a budget and set aside quarterly tax payments. Most locum tenens physicians hire an accountant or tax advisor to help them with the paperwork.

On the surface, some of these arrangements might sound like a bit of a hassle. But once they’re in place, it doesn’t take much effort to keep them going. For most people, the freedom that comes from being an independent contractor is more than worth it. Take the example of Dr. Simran Singh, a neurologist. “I have colleagues who are working their 60-odd hours a week and are also on call,” she says, in a story about her own locum experiences. “I don’t do that. I create my own schedule. I want to keep working my 14 days or fewer a month, and I can do that with locum tenens.”

Working with a locum tenens agency

Finding your first locums assignment can be tricky, but even more so if you try to do it on your own. You would have to contact medical facilities yourself. And, once you find an opportunity, you’ll need to negotiate the contract. There’s no reason you can’t do all of this yourself, but working through an agency can simplify the process.

In reality, most experienced locum tenens physicians probably recommend going with an agency at the start. An agency will find an assignment for you and help you make a successful transition.

Of course, not all agencies are equal. Some may be better at working within your specialty or be particularly strong in a certain region or country. If you have more than one specialty or professional interest, you may want to work with multiple agencies to make sure you’re getting access to the kinds of assignments that interest you.

Some doctors, like Dr. Joan Pellegrini, a trauma critical care physician, recommend going with large agencies over smaller ones because the large agencies seem to have been in the game longer. “They’re better qualified to guide you through everything you need to know,” she says.

At the same time, you’ll want an agency that watches out for its clients. A good agency will provide you with medical malpractice insurance, help you stay up to date with your licensing and credential requirements, and even cover the cost for your travel and housing while on assignment.

Just as important as the agency, of course, is the agent. Be on the lookout for someone who is a good listener and genuinely tries to get to know you—someone who you feel will truly represent you and advocate on your behalf.

For Dr. Neha Janakiraman, an emergency medicine physician, this last part is perhaps the key ingredient to securing a locums assignment that works for her. “My recruiter meets me at exactly my level,” says Dr. Janakiraman. “She’ll say, ‘Dr. J., you know they’ll love you, just be yourself.’”

How does locum tenens pay work?

Locum tenens physicians are paid by the hour, though in some cases it might be by the day. The hourly wage is good— on average $32.45 per hour more, in fact, than what permanent-only physicians earn, according to a physician pay study conducted by CHG Healthcare.

Exact numbers vary by location, and a rural hospital where physician shortages are the norm will almost always pay more than a position at an urban facility. The highest paying states for locum tenens physicians are Kansas, Nebraska, Missouri, South Dakota, Tennessee, Idaho, Wisconsin, Kentucky, Iowa, and Delaware.

Choosing your first locums assignment

Dr. Nicholas Kusnezov, an orthopaedic surgeon, has been working locums for a number of years. In the beginning, he says that he accepted almost everything. Gradually he learned what to look for, and today he says that he rejects the vast majority of the assignments he comes across. Not that the assignments are bad or poorly arranged. He’s just become more deliberate and conscious of making locum tenens serve his purposes.

To appropriately evaluate an opportunity, Dr. Kusnezov recommends asking about job composition. For example, is the assignment simply call coverage? Or is it working for a full-time practice with clinic and OR?

Living in an exotic or scenic location might sound good. But according to Dr. Kusnezov, it’s important to consider the accessibility of the place as well. How far away is the nearest airport, for example, and how often does the weather affect travel? This could be a problem if you expect to travel much during your assignment.

Urban locations can likewise be accessible or not so much. How far away will you have to live from the hospital or clinic where you plan to work? Will you be relying on public transportation or driving your own vehicle?

Finally, Dr. Kusnezov recommends finding out about case mix and patient volume, “both of which may often be overlooked when choosing a locum tenens assignment,” he says. “If you were looking for a lower-key assignment but are absolutely destroyed on call with high-acuity and complicated consults, this may in turn be overwhelming.”

Making locum tenens work for you

Pursuing a career as a locum tenens physician can lead to many exciting, rewarding experiences. Then again, it doesn’t have to be a career. You can use locum tenens for something as simple and short-term as a working vacation, to fill time between permanent positions, or to make extra money on an as-needed basis.

Whatever your situation, it’s at least good to know about locum tenens. In time, you may be able to take advantage of it. As Dr. Singh has said regarding her own career choices, “Life is what you make it, and I think the great thing about locums is that it allows you to create the life you want. And we are all on this planet for a very short amount of time so you might as well do what you want to do.”

Once you request that your data be erased from Darko Media Group’s databases, we have thirty (30) days to comply with your request. If it’s impossible to comply within 30 days, then Darko Media Group will respond to the Visitor’s request and let them know about the issue and also give them a reasonable time as to when their request for deletion will be honored.

Aside from rights such as a request to access, request to delete, and rectify, an EU user also has the right to place restrictions on the data processing itself. This means a user can limit certain things that Darko Media Group can and cannot do with their data. You can choose to limit the transfer of your data to third-party businesses (unless it’s essential for Darko Media Group’s basic functions).

You further have the right to file a complaint with a supervisory authority that oversees and handles issues related to the GDPR.

Lastly, it’s Darko Media Group’s duty to inform you that we only require information that is reasonably necessary to enter into a contract with you. We do not collect any unnecessary data, and any information we acquire is used for legitimate business purposes, such as growing and scaling our business or being able to provide satisfactory customer service to you and other users.

BRAZILIAN DATA PROTECTION LAW (“LGPD”)

The Brazilian Data Protection Law or the LGPD, which is derived from its Portuguese name. The LGPD is Brazil’s law on online privacy requirements and certain rights and privileges given to data subjects.

Under the LGPD, “processing” is defined as collection, production, reproduction, transmission, receipt, use, classification, filing, storage, control or evaluation of data, deletion, dissemination, extraction, modification, and communication. The LGPD applies to “personal data” that is defined as any information related to an identified or identifiable natural person. Moreover, sensitive data such as political opinion, racial or ethnic origin, religion, health, sex and more as they relate to a natural person.

Under the LGDP, the data subjects are given the following rights relating to their personal data:

  • Awareness and confirmation of the existence of data processing;
  • Anonymization or pseudonymization or removal of pieces of data that have been collected or processed without compliance with the LGPD;
  • Access to personal data;
  • Correction of inaccurate data;
  • Right to request deletion;
  • Portability;
  • Right to revocation of consent;
  • Right to request disclosure of any third parties with whom personal data is shared;
  • Access to the customer policy information and consent revocation terms and conditions.

The data subject has the right to exercise these rights with our business Darko Media Group anytime, free of charge.

As a business, we can only process personal data if there is any legal basis for processing that data. The LGPD provides approximately ten (10) legal basis for processing data. The ten grounds are:

  1. The data subject gives express consent to process the data.
  2. Data processing is necessary to comply with a legal obligation.
  3. Processing is essential to protect the life or physical safety of the data subject or another third party.
  4. Necessary to execute a contract or contract-related procedures that the data subject is a party to at the request of the data subject.
  5. Necessary to process to fulfill the legitimate interests of the controller or of the third party, except when the data subject’s fundamental rights prevail.
  6. Necessary to process in order to protect credit (refers to a credit score).
  7. You need to process to protect the health in relation to activities of health professionals or health entities.
  8. Necessary to process to carry out studies by research entities that ensure, when possible, the anonymization of personal data.
  9. Necessary to process to exercise rights in judicial, arbitration, and administrative procedures.
  10. Necessary to process to execute public policies provided in laws or regulations or those that are based on contracts, policies, agreements, or similar binding instruments.

Darko Media Group mostly uses legal basis #1 and #5 above, which are that the data subject gives express consent to process the data, and that processing is necessary to fulfill the legitimate interests of the controller or of the third party, except when data subject’s fundamental rights prevail to process personal and sensitive data collected from you.

VIRGINIA CONSUMER DATA PROTECTION ACT (“VCDPA”)

This section of the document supplements the information provided in this privacy policy. It is provided by the controller running the application and its parent, subsidiaries, and affiliates if applicable, who will collectively be referred to as “we,” “us,” or “our”.

The provisions in this section apply to all users who are consumers residing in the Commonwealth of Virginia and are governed by the Virginia Consumer Data Protection Act (VCDPA). If there are any conflicting provisions in the privacy policy, the provisions in this section will supersede them.

The term “personal data” used in this section is defined in the VCDPA as any information that is linked or reasonably linkable to an identified or identifiable natural person. Va. Code Ann. § 59.1-575. Personal data does not include de-identified data or publicly available information.

Categories of Personal Data We Collect

The categories of personal data that we have collected include identifiers and internet information such as NAMES, LAST NAMES, EMAIL ADDRESSES, PHONE NUMBER, IP ADDRESS. We do not collect sensitive data and will not collect additional categories without informing you. Sensitive data includes, but is not limited to, information on racial or ethnic origin, religious beliefs, mental or physical health diagnosis, sexual orientation, and citizenship or immigration status.

Reasons for Processing Personal Data

We will not use your personal data for purposes that are incompatible with those disclosed initially without your consent. You can grant, deny, or withdraw your consent at any time using the contact details provided in the document.

Sharing Your Personal Data With Third Parties

We share your personal data with third parties listed in this privacy policy. These third parties are grouped based on the different purposes of processing. For our purposes, a “third party” refers to a person or entity that is not the consumer, controller, processor, or an affiliate of the processor or the controller, as defined by the VCDPA.

Sale of Your Personal Data

The sale of personal data is defined as any exchange of personal data for monetary consideration by us to a third party, as defined by the VCDPA. Note that disclosing personal data to a processor who processes personal data on behalf of a controller is not considered a sale.

Our use of your personal data may be considered a sale under the VCDPA.

Opting-Out of the Sale Of Your Personal Data

You have the right to opt out of the sale of your personal data. To exercise this right, you can contact us at any time using the contact details provided in this privacy policy.

We will use any personal data collected from you for the sole purpose of complying with your opt-out request.

Processing of Personal Data for Targeted Advertising

We do not process your personal data for targeted advertising. If we decide to do so in the future, we will inform you and give you the right to opt-out.

Privacy Rights Granted Under the Virginia Consumer Data Protection Act (“VCDPA”)

Under the Virginia Consumer Data Protection Act, you have the following rights regarding the processing of your personal data by us:

  • The Right to Know: You have the right to know if we are processing your personal data and to access it.
  • The Right to Correct:: You have the right to request correction of any inaccurate personal data we maintain about you.
  • The Right to Request Deletion: You have the right to request the deletion of your personal data.
  • The Right to Portability of Data: We will provide a portable and usable copy of your personal data if it is technically feasible.
  • Opt-out of Targeted Advertising, Sale of Personal Data or Profiling: You have the right to opt out of the processing of your personal data for targeted advertising, the sale of personal data, or profiling.
  • Non-Discrimination: We will not discriminate against you for exercising your rights under the VCDPA. However, if the personal data or sale is necessary for us to provide goods or services, we may not be able to complete the transaction if you refuse to provide the data or ask us to delete or stop selling it.

How and When We Are Expected to Comply With Your Request

We do our due diligence to respond to all requests as soon as possible, within 45 days of the receipt of the request date. Should we need more time to respond, we will contact you and notify you of our reasons for needing more time and how much time we need. Under the law, we may take up to 90 days to fulfill your request.

We will do so if there are reasonable and lawful grounds for denying your request. You have the right to appeal the denial. Within 60 days of the receipt of the appeal, we will notify you in writing of any action taken or not taken in response to your appeal. If your appeal is denied, you may contact the Attorney General to submit a complaint.

Information Collected Through Third-Party Sources

During the time you visit and use our Site, certain limited data are collected from public databases, marketing partners, social media platforms, and analytics sources.

The types of data collected about you from other sources are your location, your computer system, which pages you have visited on our Site, how long you spend on each page, your IP address, your country, and possibly even your social media profiles and referrals.

CHILDREN’S PRIVACY AND DATA

This Website is not intended for children under the age of 13. We and this Website do not knowingly and intentionally collect any personally identifiable information from children under the age of 13. If you are under 13 years of age, please do not use or provide any information on this Website. Do not use any third parties that might have links present on this Website. Do not provide your name, address, phone number or any payment information.

If a parent or guardian believes that this Website unknowingly collected personally identifiable information from a child under the age of 13 in its database, please contact us at once at team@docsoutsidethebox.com, and we will do our best to immediately remove any and all such information from our database.

HOW DO WE USE THE INFORMATION WE COLLECT FROM YOU?
